Social Worker/Case Manager #16-56

CMP – PT, Oahu

The Social Worker/Case Manager provides professional counseling and case management services for client/residents who are 55+ years old.  Position involves evaluating and assessing potential clients’ needs, linking them to appropriate services and/or assist with placement as needed.  Employee must also be able to travel to prospective clients’ and referral sources sites, conduct regular office/home visits and visits to vendors’ business locations.  The Social Worker/Case Manager must demonstrate an active commitment to Catholic Charities Mission and strives to incorporate the four Core Values in all aspects of daily work.


Job Requirements:

Must have a Masters degree in Social Work from an accredited university plus a minimum of three (3) years experience working with the elderly population.  Must to be able to obtain a LSW within the first year of employment. Full time schedule is preferred, however part time will be considered.

Clearances Required:

Applicable background checks; HI State criminal check, TB clearance, valid driver’s license, copy of no-fault automobile insurance, traffic abstract, access to insured vehicle are required post job offer.  HI LSW, CPR/First Aid.
