Case Manager #17-22

The CCSS-VCMU provides case management services to children and families who have been reported to the Department of Human Services (DHS) when a determination has been made at intake or during on-going assessment by DHS that the child has not been substantially harmed, but is at risk of abuse or neglect by the acts or omissions of their parents or caretakers. The Case Manager assesses the family for risk of abuse/neglect, writes up an assessment, develops the service plan, refers the family for services, monitors family progress, monitors children in out-of-home placement, provides court testimony, makes the determination when the case should be closed and determines if there are safety issues that warrant returning the case to DHS for more intensive services.
